SCAIHS Application
All SCAIHS applications and accompanying forms are in fill-able format and may be completed on your computer then printed, signed, and mailed. The completed forms cannot be saved to your computer so please print a second copy for your family’s records.
Full Membership Application (For New Applicants only; Renewing Members scroll down to the Renewal Application)
- SCAIHS Full Membership Application 2012-2013 (four pages; one per family -PDF file)
- Student Enrollment Form (one per student in six-year-old kindergarten through grade 12)
- K5 Student Enrollment Form (one per 5-year-old student)
- Records Release Form (one per student in grades 2-12)
- Proposed Course of Study (one per student in six-year-old kindergarten through grade 8)
- High School Statement of Intent Form (one per student in high school; one for a 7th or 8th grade student taking high school courses for credit)
- High School Course Schedule (one per student in high school; one for a 7th or 8th grade student taking high school courses for credit)
- High School Course Description (one for each course taken for high school credit)
- High School Honors Course Addendum—for any high school course which is to be an honors course
- Credit Requirements chart
High School Enrollment Deadlines—September 15th for first semester and January 15th for second semester. Contact SCAIHS for possible exceptions at (803) 454-0427 or email (scaihs@scaihs.org) (No seniors may be enrolled after September 15th.)
Preschool/Kindergarten Application (For families enrolling only 5-year-old students or younger)
[If older students are being enrolled, please use the Full Membership Application. 5-year-old kindergarten students listed on a Full Membership Application need a Kindergarten Student Enrollment Form but are free. No enrollment form is needed for preschool students; just list them in the Family Information chart.]
